Dr. Koo and the team at Jahyun Koo DDS believe that going to a new dentist should be an easy process. We want to make your first visit with us as stress-free as possible. So your Seattle patient forms are available online.

There’s no need to fumble around with papers and insurance cards when you come in. We’ve made it easy to take care of all the preliminary paperwork ahead of time – at your convenience. Just fill out these forms, print them out, and bring them with you to your appointment. Please note that you must provide 48 hours notice for a canceled appointment. We will charge a $75 fee for a no-show appointment.
